There is no document needed for your application, any document you will need will be after you've been offered admission, we will give you the list of document that you must have after you've been admitted. 

In your registeration, all you need is to give your cyber cafe your correct details such as

1.Personal details

2. Examination details ( o level and jamb )

 3. ND detials

So this is all your cafe will take from you.
